Patient Navigation

Health Quest is pleased to be partners in patient navigation with The American Cancer Society (ACS) and share the goals of saving lives from cancer, eliminating barriers to care and ensuring timely delivery of services. The Health Quest Clinical Navigators and American Cancer Society Navigators collaborate to avoid duplication and improve service delivery.

Health Quest and American Cancer Society Navigators help to ensure that quality, safety, compassion, respect, confidentiality and professionalism are the norm for mutual patients from screening through diagnosis, treatment and through to survivorship.

The role of the American Cancer Society Navigator is to listen, list and address the patient's and caregiver's non-medical needs. They also collaborate with the Health Quest Clinical Navigators, patients and caregivers to identify and link them to available resources from hospitals, local community agencies and the American Cancer Society.

The types of services provided by the American Cancer Society Navigators are:

  • General information
  • Information and navigation
  • Look good, feel better program
  • Reach to recovery
  • Man 2 Man
  • Patient education programs
  • Transportation of all types
  • Financial assistance

The types of services provided by the Health Quest Clinical Navigators are:

  • Medical advice/referrals
  • Emotional, psychological or spiritual advice
  • Crisis counseling
  • Fundraising
  • End-of-life counseling
  • Grief counseling
  • Legal counseling
  • Financial counseling
  • Facilitation of support groups
  • Assistance with advance directives
  • Abuse and neglect issues
  • Direct recommendations on treatment options
  • Bereavement counseling
  • Procuring medical equipment
  • Transportation
