Case Management Services

WHO we are...

Professional nurses, social workers and other health care specialists staff the medical center's Case Management Department and provide a range of counseling, advocacy, discharge planning and utilization management services to patients and their caregivers. 

WHAT we do... 

Case managers are available seven days a week to help in-patients arrange for follow-up care needs (in rehabilitation facilities or nursing homes, through home care agencies, etc.).  They also service out-patients in our Poughkeepsie, Fishkill and Kingston treatment sites and maintain extensive listings of community resources available to anyone interested.

Staff organize and lead a range of education programs and support groups (see separate listing under "News and Events").  Through the adjunct Vassar Counseling and Consultation Services, they offer individual and family counseling, focusing on coping with the impact of illness and stress management. 

Internships are offered to bachelors- and master-level students in social work, nursing and related human services field from several regional universities. 

WHERE to find us...

The departmental office is located on the third floor of the main hospital building (left turn from the South Circle elevators) and is open 7 days a week (6:30 am - 4:30 pm weekdays and 9 am to 4 pm on weekends).
