Our friendly staff members will help guide you through the scheduling process. Imaging studies and reports are individually reviewed by our physicians prior to your visit. In order to provide you with a personal comprehensive treatment plan, these studies should be submitted in advance of your visit. While breast cancer does not represent a medical emergency, it is an emotional emergency, and we will make every effort to schedule your appointment in a timely fashion, allowing you the opportunity to ask questions, understand your tailored plan, and receive the highest level of breast care.

2. Preparing for Your Initial Appointment

Five Days Prior to Initial Visit:
		Prepare to bring to the office your current mammograms and/or ultrasounds along with the printed reports. These can be obtained by contacting the facility where your studies occurred. If your films were taken at Vassar Brothers Medical Center or Dutchess Radiology Associates (DRA), the office will be able to obtain these records for you. The Authorization for Medical Release form can be used as needed.
On The Day of the Initial Visit Please Bring:
		Completed and signed Patient Information Form and Comprehensive Patient History Form as indicated in your New Patient Welcome Letter (English) / New Patient Welcome Letter (Spanish). Please arrive 30-minutes early if you have not completed necessary forms prior to appointment. Your insurance card and drivers license. Your insurance referral (if needed for appointment). Your co-payment.
