Advanced Care Planning

Vassar Brothers Medical Center now offers free Advanced Care Planning. 

Trained facilitators are available to meet with anyone interested in learning more about Health Care Proxies, Living Wills, Do Not Resuscitate (DNR) / Do Not Intubate (DNI) documents or any questions regarding future health care needs. 

While the service is free, appointments are required and can be made by calling the Case Management Office at (845) 437-3101.

Meetings are held at the Counseling and Consultation Office on the first floor of the hospital.

Referrals from private physician offices are welcomed.
